Personal Information Voluntarily Submitted to HHS
If you choose to provide HHS with personal information -- for example by completing the respirator questionnaire -- we will use that information to implement the procedures for approving the use of respirators at work. These are described in the Occupational Health and Safety Administration’s (OSHA) Respiratory Protection program (OSHA Title 29 1910.134). OSHA guidelines for medical evaluation records require us to retain the information for the duration of employment plus thirty (30) years. It may be subject to disclosure in certain cases (for example, if lawfully required in response to a court order, or Congressional access request, or if authorized by a Privacy Act System of Records Notice). The data is subject to the Privacy Act since it is maintained in a Privacy Act system.
